Abstract
US, and especially African American residents in US major urban areas, have lower life expectancy and worse health outcomes than residents living in outer US suburbs and other high-income countries. This is true even with exorbitant spending on health care which is nearly twice as high as costs among other high-income countries. Research on the US health burden noted proximal causes, such as opioids, firearms, and obesity. The disadvantage has led scholars to examine systemic factors that may be the root cause of the issue. Experts convened by the National Academy of Medicine and National Research Council identified explanations in five realms: health care, socioeconomic conditions, behaviors, the environment, and public policies. In this article we examine the global phase-out of leaded petrol and the ongoing anthropogenic global warming as possible causes of lower life expectancy and depressed health outcomes. These two factors are often treated as distinct environmental issues separated by time, mechanism, and policy response. However, both phenomena share a common origin in the petroleum industry and exhibit striking parallels in development, scientific recognition, and regulatory trajectories. This article synthesizes historical, environmental, and public health evidence to argue that the widespread use of tetraethyl-lead in petrol and the combustion of fossil fuels represent two interconnected environmental health crises that unfolded across a single century. Both involved early awareness of potential harm, prolonged industrial expansion, and delayed regulatory intervention, resulting in global-scale exposure and persistent health consequences. By integrating these narratives, this article proposes a unified conceptual framework that combines leaded petrol and fossil-fuel emissions within a broader pattern of industrially mediated environmental risks. This perspective highlights the persistence of environmental contamination, the amplification of social inequalities, and long-term biological and societal impacts that continue to shape population health today and into the future.
